From The Oregonian, page C7, June 30, 2005:

A cyclist was killed Wednesday night in unincorporated Multnomah County near the Springwater Trail. Multnomah County sheriff's spokesman Lt. Mike Shults said the woman was hit by a Ford 350 pickup just before 7 p.m. She was pronounced dead at the scene. The woman was carrying no identification, was not wearing a helmet and was listening to a CD player, Shulz said. She was crossing Southeast 252nd Avenue near the intersection with Southeast Telford Road when she was struck. The driver of the pickup is cooperating with the investigation, Shults said.

According to a witness,

what the newspapers failed to report is that there was a tree in front of the stop sign on the trail. Also the blackberries had grown up so as to limit the visibility. Yes, she was wearing headphones and didn't have a helmet but the poor visibility was also a huge contributor to the accident.

Brenda's friends and family presented at a meeting of Gresham's Parks and Recreation Council Advisory Committee:
Four friends and family of Brenda Elliott, the cyclist who was fatally injured in an accident on the Springwater Trail, brought up safety issues. They suggested that the entire trail be evaluated for safety issues, and expressed their opinion that the trail was very dangerous due to inadequate markings at intersections. One commented “Even police didn’t know that the speed limit was 25 MPH through the crossings”. They want to see consistency on the markings, as there are many that are different. They suggested posting a speed limit sign of 25 MPH within 1000’ of driveway. They would like to do a memorial for Ms. Elliott. Staff indicated that all of these suggestions will be evaluated by an inter-agency group being formed.

Four cyclists were killed in a little over a month in Portland and the surrounding areas in the summer of 2005.
